Policy Area Health – Improving and promoting people’s health, including its social aspects

The aim of the main project proposal is to reduce significant organisational and management inefficiencies in the care system. It intends to increase the efficiency of the care systems by reducing the hospitalisation rate for ambulatory care sensitive conditions (health conditions for which hospitalisation or emergency care can be avoided through addressing these conditions effectively in primary health care level). The project intends to strengthen the primary care, especially the role of nurses in primary care teams. It will facilitate teamwork and cooperation with public health and social care sectors and to enable patients to better manage their health needs.
