#S26 Integrated care for senior citizens around the Baltic Sea (INTEGBALT)

PA Health – Improving and promoting people’s health, including its social aspects
International experience and evidence show that there is scope to broaden and improve home-based care by joining the efforts of primary health and social service teams. To achieve this potential, care in local hospitals and residential homes should be synchronised as much as possible. The main project will aim at finding solutions for how countries at different phases of development could advance through a joint project towards these objectives. The expected outcome would be a sustainable integrated system of care. Such a system would combine efficient use of resources with the expectations of patients with chronic illnesses to receive care at home and remain as independent as possible.
